Patient Health Questionnaire (PHQ-9)

This 9-question self-assessment helps identify signs of depression and assess its severity. It’s a helpful first step toward understanding your emotional health—your responses are private and not a formal diagnosis.

The PHQ-9 (Robert J. Spitzer, Dr. Janet B.W. Williams, Dr. Kurt Kroenke, and colleagues from Columbia University, 1999) is a brief the nine item self-administered depression scale. It can be useful in the identification of problem symptoms associated with depression.
